Steps to Complete Your Evaluation

Request Appointment
Call or email to schedule your evaluation. You can expect an appointment date the same day you call.

Records Review
Before your appointment, we will request to review prior treatment records, medication history, or related documents to provide a full picture to the FAA.

Comprehensive Clinical Interview
You’ll meet with a board-certified psychiatrist familiar with FAA protocols. We’ll discuss your medical history, work experience, mental health background, and any recent life changes.

Psychological Testing
If necessary, standardized assessments may be recommended to provide additional data and context. These tools are selected based on FAA guidelines and your specific circumstances.

Opportunity for Feedback
You will be sent your report to allow the opportunity for feedback before submission.

FAA-Compliant Reporting
Following the evaluation, we prepare a detailed report in the FAA-required format. Our reports aim to be objective, thorough, and submitted in a timely manner, usually within 2 weeks.

Recommendations
If any recommendations are warranted after your evaluation, you will receive a list with explanations provided by Dr. Lubinsky.
Rates and Timeline
Rate: 300/hour
120 minutes average length of interview portion (face-to-face)
Record review and report portion - 6-12 hours depending on FAA requirements.
An estimate of final costs will be given to you before scheduling, however depending on findings, additional time might be required for a thorough report.

Our office is located in a modern medical building designed with comfort, privacy, and accessibility in mind. From the moment you arrive, you’ll find a quiet, professional environment. The space is fully ADA accessible, with elevators, wheelchair-friendly entrances, and accessible restrooms to ensure every person feels welcome and accommodated.
Flying In? Traveling from Denver International Airport (DEN) to Fort Collins is straightforward, with several convenient options to suit your preferences and schedule.
Driving
The drive is approximately 70 miles and takes about 50 minutes to 1.5 hours depending on traffic.

Record Requests
To ensure a thorough and timely FAA psychiatric evaluation, we kindly request that all relevant medical records be submitted in advance of your appointment. These may include previous psychiatric or psychological evaluations, treatment summaries, hospital records, medication history, and any FAA-related correspondence or documentation.
Records can be securely faxed, mailed, or uploaded via our HIPAA-compliant patient portal. Providing complete and accurate records helps us meet FAA reporting requirements and ensures that your evaluation is as efficient and comprehensive as possible.
